Forms

  • Travel Forms. The Travel forms are in acrobat pdf format. Indicates a form that may be filled in by computer. Type the information in the blanks provided, print using your pdf print icon, and sign where indicated.

    Note that you can [Tab] from one blank to the next and [Shift][Tab] back or use your mouse. Some blanks have been preset but they may be changed by retyping. Repeated fields (like your child's name) are designed to self-fill.

    Start with page 1 and complete the form data by filling in the required spaces. The best way is to tab between one field and the next. The check the pages and print them out. Sign every page.

  • Other Forms
    • Planned Absences. You will need to complete this form for absences from planned rehearsals or performances. Planned absences should be reported at least two weeks in advance.

Troubleshooting

You should have Acrobat 7.0 reader as a part of your web browser. If not, use Acrobat's free download. You will need at least Acrobat 5.0 in order to fill in forms. If you prefer, you can always print and fill in the blanks.

If you have trouble downloading the forms try the following.

  • Make sure you have Acrobat Reader installed on your computer.
  • When you click on one of the forms links, your browser begins by loading the Acrobat Reader. You will see the Acrobat flash screen pop up to advertise that Acrobat is at work. If this doesn't happen then you either don't have Acrobat properly installed (install it) or you have insufficient resources to run it (shut down programs that may be running in the background).
  • Hold down the shift button when you click on the link. This will open a second window for the downloaded document. Then if the new window has difficulty downloading you can hit refresh. This often restarts the process.
  • The forms package is fairly big; it will take two or three minutes to download. If you are unsure of your ability to read the forms start with a smaller file like the basic field trip permit.
